Frederick Holmes School works with the students and their families to ensure a smooth transition and to prepare them for life once they leave us. The transition planning is about equipping the young people for adult life and ensuring that they are aware of the various provision available to them when they leave Frederick Holmes, that can meet all of their needs.
The students are encouraged to be part of the planning process and they begin doing this as soon as they transition into the college centre. The students carry out accreditation challenges where they are supported to think about their future goals and identify ways in which they can achieve these goals. The students work with the Connexions PA and the class team, to prepare for their person-centred Education Health Care Plan (EHCP) Annual Review Meetings.
From the age of 14, the EHCP review meeting includes a transition outcome to prepare pupils for moving on. The transition outcome may consider daytime activities, living arrangements, leisure opportunities, health care and learning needs such as increasing independence and experience of other providers. The students and their families are supported throughout this process by Ann Yates, the school’s family support officer.
During the transition years there is an increasing emphasis on experiencing the world of work and enjoying links with other environments including schools, colleges, day care providers and community facilities such as libraries, leisure facilities and shops. The students are also supported to visit a range of provisions and once a suitable provision has been identified we can organise some taster days to visit the providers to endeavour to reduce any anxieties that both the students and their family may have.
We understand that transitioning can be an anxious period for everyone but the school will help to signpost families to other organisations that can offer support and also the staff at Frederick Holmes are available to provide support throughout this journey.
